ICHRAs Offer Small Businesses a Way Out of Group Health Insurance

Small businesses are increasingly turning to individual coverage health reimbursement arrangements, known as ICHRAs, as an alternative to traditional group health insurance. Instead of selecting and managing a single plan for all employees, an employer using an ICHRA sets aside a fixed dollar amount that each worker can use to buy their own individual health coverage on the open market. Employee Benefit News reported on how small business owners are restructuring benefits this way to keep costs predictable and plans accessible.

What an ICHRA actually does

Under a traditional group plan, the employer picks the insurer, the network and the benefit design, then splits the premium with employees. An ICHRA flips that: the employer decides how much money to contribute, and each employee shops for their own individual policy. The employer's cost is capped at whatever allowance they set. The employee's cost depends on what they choose.

The model can make budgeting simpler for a small business owner who cannot absorb a surprise renewal increase. Medical care prices were up 1.5 percent compared with a year earlier, according to the US Bureau of Labor Statistics figures for September 2026, which is well below the 3.7 percent rise in consumer prices overall over the same period. Even so, group health premiums have historically moved on their own logic, and a small employer with a handful of workers can see rates swing sharply after one bad claims year.

Where the model creates risk

The problem Employee Benefit News identified is what happens when employees are handed a budget and told to shop. Individual market exchanges in most states list hundreds of plans. Deductibles, networks, drug formularies and out-of-pocket maximums vary enormously, and a worker who picks the wrong plan can end up with coverage that does not cover their doctors or leaves them exposed to large bills.

Marshall Darr, chief executive of StretchDollar, a company that works with small employers on ICHRA administration, put the structural gap plainly: "[Large] companies have a whole infrastructure that these smaller employers just functionally do not have." A large employer typically has an HR department, a benefits manager and a broker relationship that guides employees through open enrollment. A ten-person shop has none of that, and the ICHRA model does nothing by itself to fill the gap.

What small employers and their advisers should watch

The ICHRA structure shifts underwriting risk from the employer to the individual insurance market, which is a meaningful change in how liability is distributed. An employee who selects a plan with a narrow network and then needs a specialist outside it has no recourse to the employer, because the employer's obligation ended when the allowance was funded.

That is not necessarily a problem, but it does mean the quality of the decision an employee makes at enrollment has direct consequences for them, not for the business. Employers who adopt ICHRAs without building in some form of decision support, whether through a broker, a benefits platform or a navigator service, are effectively transferring a complicated financial decision to workers who may have no experience making it.

For small business owners and their advisers evaluating the model, the practical question is not whether an ICHRA is cheaper than a group plan in year one. It is whether the employees who will use it have enough support to choose coverage that actually protects them. The arrangement's cost predictability is real; the adequacy of what employees end up with is not guaranteed by the structure itself.
